Insurance Questions

  • What insurance plans do you accept?

    We accept most insurance plans and commercial insurance plans, including Sanford Health and BlueCross BlueShield. We also accept Medicare, PPOs, HMOs, worker’s compensation insurance, and more.


    Please be aware that insurance programs differ greatly in their rules regarding treatment and coverage for physical therapy and occupational therapy services. You are responsible for any copays and services not covered by your insurance.


    ProRehab Center is not a Medical Assistance (MA) provider.

  • What if I don’t have any health insurance?

    We offer reasonable rates and payment options when you’re uninsured or under-insured. During your initial, complimentary consultation, we’ll work with you to arrange self-payment options.

Referral Questions

  • Do I need a referral from my physician to see you?

    It depends on your insurance plan. Most Blue Cross Blue Shield of ND policies do not require a referral for therapy. Please check with your insurance provider or contact our office to find out if you need a referral.

  • My physician referred me to therapy at another clinic, but I’d like to come to ProRehab.

    If your doctor prescribes therapy, simply exercise your right to ask your physician to refer you to ProRehab. We will work with any physician to keep them updated on your progress, and we’ll keep you informed if there are any differences in your insurance coverage.

  • What is the difference between a CONSULTATION and an APPOINTMENT?

    We offer a free consultation so you can meet with one of our physical therapists or occupational therapists and determine if physical or occupation therapy is a good idea for you. No referral is needed.


    If you begin therapy, we will schedule an appointment for a thorough evaluation and to start your treatments. These appointments come with a fee that will be submitted to your insurance company.

Network Questions

  • Is ProRehab affiliated with a network?

    We are proud to be a member of the Heartland Healthcare Network, an alliance of more than 250 independent health care providers.

  • What if my insurance plan is part of a medical network?

    If your insurance plan indicates a specific medical network, we’ll help you determine if there is a difference in your coverage rates and how that will affect what you pay for our services.

Appointment Questions

  • <>How do I set up a consultation or an appointment for therapy?

    Either call our office or ask your primary health care provider to call and make an appointment in your name.


    Follow-up appointments range from 20 minutes to 1 hour, depending on what type of treatment you and your therapist decide on.

  • <>What will happen at my first visit?

    We schedule the day of the initial visit depending on what the therapist would like to do- between 1 visit or multiple visits up to 3x a week for 4 weeks.  


    Please arrive about 15 minutes prior to your first appointment and plan on an hour and 15 minutes for your first visit. (Follow-up appointments will usually last an hour.)


    A typical first appointment will proceed as follows:


    Paperwork:

    You will need to complete some paperwork covering your medical history and insurance. (If you’d like to complete it before you arrive, please locate it on our Patient Resources page.) We’ll also make a copy of your insurance card and ask for a list of any medications you are currently taking.


    Exam:

    A comprehensive evaluation of your overall health will include discussion about any health concerns or chronic conditions. We will assess your physical capacity, range of motion, strength, and other factors. Please be sure to wear comfortable clothing that allows free movement for bending, stretching, and position changes. Ladies should avoid wearing skirts or should bring a pair of shorts or pants to change into during the visit.


    Treatment Plan:

    Following your health evaluation, your therapist will review the information and develop your personalized treatment plan. Depending on how long it takes to get any relevant testing results or medical records, this could take up to two weeks. Once the therapist has created a therapy plan, we will call to schedule your next appointment, which will include a thorough review of the treatment recommendations and the start of your first therapy session.
